Chesapeake Chapter of the U.S. Lighthouse Society – Preservation Volunteer Form

Instructions for submission:

1. You must be a Chapter member to become a volunteer and participate in Chapter projects. If you are not a member become one today by following this link to the membership form.
2. FILL OUT FORM. Fill in all required fields in registration form below.  Or complete only step 3 to print a blank form to give a friend.
3. PRINT. Click the print button on the upper right corner of the page, make 2 copies, one to send and one for your records.
   NOTE: If you click the submit button without printing first you will lose all the information entered.
4. SUBMIT. Click the submit button, your form will be sent electronically to our Preservation Director.
