WE PREFER THAT YOU ORDER YOUR MEMBERSHIP THROUGH OUR LOFT OR THE MEMBERSHIP PAGE.  It saves our volunteers a significant amount of work when you do. 

The Paid Life Membership and the 5-Year Membership plans are NO LONGER available.  Please do not use this manual form to order those.

Instructions for manual submission of the Membership form:

1. Download the the Membership flyer with order form by clicking  HERE.
2. Fill out the form. 
3. Make you check payable to:  Chesapeake Chapter, USLHS
4. Mail the completed form and check to:  Chesapeake Chapter P. O. Box 178  Joppa, MD 21085-0178
